We are currently entering the adapting stage in the condition of Zauji's right eye affected by retinal ablation. A retinal ablation is a condition of detachment of the retina, or the membrane of the eye, from its original position that can cause permanent blindness because the retina lacks oxygen and nutrients. The experience of having retinal ablation surgery seven times in Cicendo Bandung Hospital makes us pay more attention to the pressure of the eyeball that must be maintained at <24.
In the previous control, Zauji’s eyeball pressure rose to 40. The glaucoma section advises laser action to lower the pressure of the eyeball. After a routine of dribbled timol and glaucoma, 3 days later the eye pressure was relatively stable in the 20s range.
The
retina says silicon evacuation is not done, and we are welcome to
control the glaucoma until the eye pressure is stable.
Eyeball Pressure
Our concerns are reduced because the results of the virus every month that we do in the pavilion clinic show the pressure of the eyeball remains normal. Giving glaucon stopped
because continuous use will cause inflammation so that the timolol
becomes the only drug to keep eye pressure stable.
For one year
and four months, we have been residents of Mata Cicendo Hospital,
Bandung. Still in
the period of large-scale social distancing (PSBB) in the city of
Bandung, the screening system began to be simplified when registration began. When in the early pandemic, we had to queue for a long time to undergo Covid-19 . In advance, at the beginning of this year, the screening was done online
for patient
control and direct screening for new patients.
At
the beginning of this year, Prof. Arief S. Kartasasmita, dr., SpM (K),
M.Kes., M.M., PhD, the retinal subspecialist doctor, stated that Zauji's
eye papil began to pale. The pupil is in the middle of the eye and is round and black, which serves to
capture light and focus on the retina as the center of vision.
Hasil Optical Coherence Tomography (OCT)
A pupil that starts to pale can be caused by many things.
Among them are glaucoma and the presence of foreign objects (silicones) in the eyes. Based on previous retinal detachment surgery experience, the option of
Prof. Arief is the evacuation of silicon. But beforehand it must be ascertained that the pencil is not
It is caused by the glaucoma effect.
This
is an important note for us because it means that we have to return to
consult the glaucoma section with Dr. Andika Prahasta, SpM(K), M. Kes, or Dr. Maula Rifada, SpM(K).
Prof. Arief also suggested doing an OCT papil examination. Optical coherence
tomography (OCT) is a medical diagnostic imaging technique.
Take advantage of photonics and fiber optics to get images and
characterize eye tissue.
OCT
As
always, even though the consultation is done at the pavilion clinic,
for actions such as OCT examinations, we will use BPJS services in
regular clinics. The next day we went back to the glaucoma section and
did
OCT according to the queue at the regular clinic of BPJS.
Examination
of a glaucoma doctor at a regular clinic mentions that in the case of
Zauji, papil paled.
It cannot be specifically known the cause. Because although Prof. Arief
predicts the pale-colored papil due to the effect of silicon, the glaucoma
part states pale papil is also common in
people who have high minus.
However,
the glaucoma doctor gave consent, or ACC, to the evaluation of silicon when it is desired by the retina. The experience
of retinal ablation surgery that we have undergone makes us better
understand the examination path that seems back and forth. However, an
action must be prepared carefully, especially physically and emotionally;
our strength is limited. OCT results must also be consulted first with Prof. Arief in the retinal clinic.
