Cancer Treatment After Chemotherapy

2022 became the second year her mother underwent cancer treatment. Moving from one hospital to another, from one doctor to another, we have made this effort to complete cancer treatment after chemotherapy was completed in August 2020.

Breast Cancer Treatment Circuit

When you said there was a lump in the left breast, I immediately registered him in the general surgical section of Hermina Arcamanik Bandung Hospital. After biopsy surgery, the doctor recommends a direct mastectomy because the results of the diagnosis indicate the presence of carcinoma or cancer cells.

The mother was included and not fussy in the treatment so that the mastectomy was carried out a few weeks later, which continued with chemotherapy at Santosa Kopo Bandung Hospital even though the results of the mastectomy showed there was no spread of cancer in the lungs and spleen. Chemotherapy that lasts for 4 months for 6 cycles (6 times the administration of chemotherapy drugs every 3 weeks).

Thank God your mother’s condition continues to improve with chemotherapy so that routine control can be done every month. Doctors also recommend immunostochemical examinations to ascertain the type of cancer suffered by the mother. Because immunohistochemical results showed good results until the middle of 2022, oncology surgeons only gave herbal prescriptions to maintain the immune system of the mother because the mother can do relatively normal activity.

On the advice of some people, I asked the doctor to do an additional examination in the form of an ultrasound and chest X-rays. Alhamdulillah, both show encouraging results, so the doctor refers the mother to exercise control every three months.
During treatment, you take advantage of the one-class self-service BPJS facility to cover all costs from biopsy, mastectomy, and chemotherapy, including blood tests, heart ultrasound, x-rays, and others. Some of the costs we incurred to speed up the time of the inspection.

Refine in Faskes 1

As always, when treating using BPJS, in addition to the BPJS payment bill, Menong’s friends must ensure referrals from Faskes 1 to the next Faskes are still valid. We deliberately chose Pratama Clinic near the house instead of Puskesmas with the consideration that the queue in the clinic is relatively short and it is open almost 24 hours.

The doctor in Faskes 1 does not require the mother to come directly when going to extend the referral. But because we want to tell you the series of breast cancer treatments that we have done, I invite my mother to see a doctor at the clinic.

General practitioners and administrative officers in the clinic are also very good and cooperative. The doctor also examined the scars of the mastectomy and confirmed they were in good condition.
Surprisingly, the officer in Faskes 1 had difficulty registering the mother at Santosa Kopo Bandung Hospital. This process can usually be done in a matter of minutes. After being tried many times and continuously rejected, the officer asked us to return in a few more days because there was an error in the system.

Unfortunately, a few days later, the system on the computer was still unable to connect with Santosa Kopo Hospital. Finally, I took the initiative to ask directly at Santosa Kopo Hospital. The security officer in the oncology surgery section said the patient from Faskes 1 could no longer be referred directly to Santosa Kopo Hospital since June 2022 due to a class type change.

Because I was still curious, I had the initiative to ask directly at the BPJS section of Stosa Kopo Hospital. Administratively, Santosa Kopo Hospital has a very neat system and provides ease of service for patients. As was the case when first registering the mother in this hospital, the special officer of BPJS served all the patient's questions face-to-face.

Clear and clear answers bring enlightenment. In order, this rejection occurs due to a change in the type of class of Santosa Kopo Hospital, whose original Class B became Class A. This means that from Faskes 1, the mother should be referred to Faskes 2 class B first. If in Faskes 2 class B can not be handled, the patient will be transferred to Faskes 3 class A.

This means that you should not be referred directly to Santosa Kopo Hospital and should find a class B-type hospital first. After looking for information about oncology surgery in various hospitals not far from home, we finally decided to continue treatment at Al Islam Hospital Bandung.

The mother herself is often treated at Al Islam Hospital Bandung because it is close to home and can be reached with one angkot only. I myself looked for the schedule of on-log surgeons and how to register my mother as a new patient.

Al Islam Bandung Hospital Onk Surgery

Since you have been treated at Al Islam Hospital, you are allowed to register by phone at 022 7506078 to get the queue number. Do not forget to mention the medical record number for the patient's data. There is only one oncologist at Al Islam Hospital Bandung, namely Dr. Rian Robian, Sp.B(K)On, FINAC, FICS, every Monday and Thursday at 14.00–16.00 WIB. In terms of time, this schedule is easier because, in addition to being close, you also no longer need to take medicine until the night.

Because it is the first time to treat in the oncological surgery section, we have to follow the patient registration procedure. The security officer gave tips so that the registration queue is not too long. Since the mother is categorized as elderly, we are allowed to take a special order number for the elderly.

We also come at 13.00 WIB, and there is no need to come in the morning (or dawn) like the BPJS queue in the hospital in general.

The administrative requirements of BPJS itself are not too much, but with a referral letter from Faskes 1, KTP, and medical record cards, Mother has obtained a queue number in the oncology surgery clinic. There are not as many oncology patients at Al Islam Hospital Bandung as at Santosa Kopo Hospital. The mother got the fifth queue number of 10 patients.

Previously, I had time to talk with other patients who have long been treated at Al Islam Hospital to find information about Rian doctors that we will meet later.

Dr. Rian Robian, Sp.B(K)On, FINAC, FICS

Apparently Rian's doctor was still young for the size of a cancer specialist. As usual, I explain the journey of Mother of A-Z, what treatment has been received by you, and your current condition. I also explained that the condition of the mother has been declared good so that she only needs to check once every 3 months.

Unexpectedly, Dr. Rian gave a statement that was quite striking and reminded me on what basis the mother was declared healthy and cured of cancer. He explained that a patient should not be arbitrarily stated to have been cancer-free just because he can move normally.

He also asked what the results of the examination that had been done after the chemotherapy were. Only the results of the stomach ultrasound and x-ray that we brought (immunocytochemical results were also forgotten; I did not bring them at that time). The doctor asked if her mother had done a bone scan and a blood test.

The doctor gave a referral so that both could be done immediately. As a consideration, I chose to use the self-help costs in the nearest laboratory because if you still want to use BPJS, both examinations can only be done at the Hasan Sadikin Center General Hospital Bandung, which definitely requires following the queue for up to 3-6 months.

The cost of a blood examination ranges from 1 to 2 million, and the results must be submitted immediately so that the doctor can follow up on the next treatment that will be given to the mother. The doctor suggested asking for a scan of the bones costing 4–6 million at Santosa Kopo Hospital for a certainty of the schedule.

The doctor also explained that although it seems healthy, the condition of the mother cannot be ascertained. Especially at Santosa Kopo Hospital, doctors never do a physical examination and are only given a prescription. Although impressed by her fierceness, we are grateful that Rian's doctor is very thorough and friendly in providing explanations.

I’m actually feeling guilty for ignoring this. But there must be a real solution. At the hospital, we immediately glided to do a blood test at the nearest PRODIA laboratory.

Although a little nervous, Alhamdulillah, blood tests after chemotherapy can be passed well. The results of the CA 15-3 and CEA tests we will get on Friday afternoon so that on Monday, Mother can return to consult with Rian's doctor. What about the results of this test? And can a bone scan be done at Santosa Kopo Hospital?
